HEALTH COVERAGE

Henry County Health Center is self insured.  The Plan is funded by Henry County Health Center and associate contributions.  Employee Benefit Systems (EBS) in Burlington, Iowa is the third party administrator. 

Henry County Health Center offers two health plans to choose from.  You may choose whichever plan best suits your needs. 

 

Health Plan B

Eligibility

Associates must be scheduled to work 30 or more hours per week and have successfully completed their 90-day initial employment period.  Coverage will begin the first of the month following the 90 day initial employment period.

Pharmacy

Prescription claims will be paid according to your major medical plan.

Health Savings Accounts

A Health Savings Accounts (HSA) is a tax advantaged trust or custodial account created for the benefit of an individual covered under a qualified high deductible health plan.  For HCHC that qualified plan is Plan C.  Funds from this account can be used to pay for qualified medical, dental, and vision expenses.  HSAs are portable and unused balances carry forward to future years.  HCHC contributes money towards these accounts for eligible participants.

Flex Credit

If you are eligible for the group health coverage and elect not to be covered under our plan due to coverage elsewhere, you are eligible to receive a credit of $40 per month that will be applied to your Health Care Flexible Spending Account.  A copy of your current health insurance card that documents you are covered or a statement from another employer or insurer documenting your coverage is required.

DENTAL COVERAGE

The dental coverage is also a self funded plan.  The dental plan does not utilize a network.  You may choose any dentist.  There are two plans to choose from.

Eligibility

Associates must be scheduled to work 30 or more hours per week and have successfully completed their 90-day initial employment period.  Coverage will begin the first of the month following the 90 day initial employment period.

 

Low Option Plan

Diagnostic and preventative services paid at 80% of UCR, no deductible.

The maximum annual benefit per individual is $750

High Option Plan

Diagnostic and preventative services paid at 100% of UCR, no deductible.

This plan includes orthodontics for unmarried eligible dependents.  Lifetime maximum is $1500 per member.

LIFE AND LONG TERM DISABILITY INSURANCE

Eligibility

Associates must be scheduled to work 30 or more hours per week and have successfully completed their 90-day initial employment period.  Coverage will begin the first of the month following the 90 day initial employment period.

Life Insurance

Henry County Health Center provides one times your annual pay in term life insurance.  All eligible associates have the option of buying an additional one to five times annual pay.  Rates are income, gender and age sensitive.

Dependent Life Insurance

Eligible associates may purchase term life insurance for dependents.

Personal Accident Insurance

Accident insurance can help you pay expenses if you or your spouse is seriously injured or killed in a covered accident.  This coverage can help ensure that tragedy doesn’t take both an emotional and a financial toll on your family.  You can select from $10,000 to $500,000 in coverage, in units of $10,000, at an affordable price.  Coverage can be purchased for associate only or associate and family.

Long Term Disability

This plan is designed to replace a portion of an associate’s income in the event of disability as a result of an accident or illness.  A 3 month period of total disability is required before beginning to receive any benefit payments.  Henry County Health Center provides eligible associates with 60% of base pay.  

FLEXIBLE SPENDING ACCOUNTS

Eligibility

Associates must be scheduled to work at least 8 or more hours per week and have successfully completed their 90-day initial employment period.  Coverage will begin the first of the month following the 90 day initial employment period.  There is no waiting period for dependent flex.

Health Care Flex

Health Care Flexible Spending Accounts give you a great way to pay for un-reimbursed health care expenses.  With Health Care Flexible Spending Accounts, you actually increase your take home pay because you are paying for those expenses with dollars that have not already been taxed!  Health Care Spending Accounts are for health, dental, and vision expenses for you and/or your dependents that are not reimbursed under your insurance plan.  Examples include deductibles and co-payments, orthodontics, chiropractic care, vision services (exams, eyeglasses and contact lenses), and health or dental expenses in excess of the reasonable and customary amount covered by your plans.

Dependent Flex

The Dependent Care Account allows you to pay for your dependent day care expenses with before-tax dollars.  Currently, if you are a single working parent or married with a working spouse, you are eligible to receive a tax credit on up to $3000 a year in expenses for the care of one dependent (or $6000 a year in expenses for the care of two or more dependents).  Using the spending account may allow you to reduce your taxes more than the tax credit already does.  You cannot, however, use both the tax credit and the spending account for the same dependent day care expenses, and expenses eligible for the tax credit are reduced on a dollar-for-dollar basis by the amount you contribute to the spending account.

(The amount you set aside annually for any spending account is binding for the entire plan year, unless you choose to change it to meet needs resulting from a qualifying change in your family circumstances.  Examples of changes in family circumstances might include marriage, divorce, birth or adoption of a child, or a change in employment status.  The change in your election must be consistent with your change in family circumstances. )

RETIREMENT PLANS  

IPERS

(Iowa Public Employees Retirement System).  All associates are required to participate in this excellent retirement program.  Associate contributes 4.3% of their gross pay every pay period and Henry County Health Center contributes 6.65%.

457 Plan

An optional tax-deferred investment plan.

Roth IRA

An optional after tax savings plan.

PAID TIME OFF 

Paid Time Off  (PTO) is for part time and full time associates.  PTO combines vacation days, holidays and short-term illness in one unit of measure, providing greater flexibility for use by associates.  The amount associates earn depends on length of employment and hours worked.
